Notice to Prospective Providers
Thank you for your interest in becoming a provider of services to the citizens of South Carolina.
Becoming a provider involves two (2) phases or steps.
- The first phase/step is to be either qualified or verified by the South Carolina Department of Disabilities and Special Needs (DDSN).
- The second phase/step is to enroll as a Medicaid provider which is accomplished through the South Carolina Department of Health and Human Services (SCDHHS). Once qualified or verified by DDSN, information that will help you accomplish the second phase/step will be provided to you.
Prospective providers are either qualified or verified based on the service they are seeking to deliver. Please refer to the information below and follow the links included to learn more about the QUALIFICATION or VERIFICATION process for each service you are interested in delivering. A separate application is required for each proposed service.
